How do I set up a page when someone wants to sub. that has specificie questions on it that needs to be answer?

have seen these pages before. etc. name, where you are from, email addy, reason for joining, affiliation groups and then a coded group of letter/numbers that have to be retyped in. all before you hit the submit to join.
